Welcome to Miami Beach Urgent Care. By accessing our services, website (https://miamibeachurgentcare.com), and communication platforms, including SMS and email, you agree to comply with these Terms and Conditions. If you do not agree, you should not use our services.

If X-rays are taken, a $15 Radiology Fee is required at the time of service.

This fee includes an independent radiologist review.

The fee is not billed to insurance and must be paid directly by the patient.

3. Release of Medical Information

You authorize Miami Beach Urgent Care to share medical records with mutually agreed-upon physicians or
medical facilities as needed for your care.

4. Agreement of Charges

You agree to pay all outstanding charges at the time of service or when billed.

The clinic cannot guarantee insurance coverage; final benefit determination is made by your health plan.

Some services, including labs and X-rays, may be subject to deductibles.

5. Credit/Debit Card Authorization (For Insurance Patients)

Miami Beach Urgent Care files claims as a convenience.

You authorize balance billing to a credit/debit card for patient responsibility amounts determined by your insurance carrier.

Card information is stored securely and processed through First Data.

Your card will only be charged for outstanding balances after insurance processing.

6. Payment Policies & Fraud Prevention

By making payments, you agree to any terms set by your card issuer.

Any attempts to dispute valid charges through false chargebacks may be considered fraud.

Miami Beach Urgent Care may take legal action, including collection efforts, reporting to consumer agencies, or civil/criminal litigation for theft of services.

False disputes will also result in the loss of credit/debit card and insurance privileges at our facilities.

7. Mobile Messaging and Communication Policy

Miami Beach Urgent Care may contact you via SMS and email for appointment reminders, billing notifications, and important healthcare updates. By providing your mobile number and email, you consent to receive these communications.

7.1 Message Frequency

You may receive messages weekly or as needed, depending on your medical care and account status.

7.2 Potential Costs

Standard message and data rates may apply as determined by your carrier.

We are not responsible for any costs incurred by your mobile carrier related to our communications.

7.3 Opt-In Consent

By providing your mobile number and email, you explicitly consent to receive SMS and email communications from us.

This includes notifications related to appointments, billing, medical follow-ups, and other healthcare-related information.

7.4 Opt-Out Instructions

You may opt out of SMS communications at any time by replying STOP

After replying STOP you will receive a final message confirming your opt-out request and you will be removed from all systems within 24 hours.

To opt out of email communications, use the “unsubscribe” link included in our emails.

Opting out of communications may affect your ability to receive appointment reminders and other important updates.

7.5 Privacy and Data Sharing

We do not share your mobile or email information with third parties for marketing purposes without your explicit consent.

Your data is used solely for healthcare-related communications and necessary operational purposes.

8. Help & Assistance

If you need assistance regarding SMS or email communications, text HELP for support.

Once you reply HELP you will receive a text including our phone number, email address and/or website and we will be able to assist you.

Miami Beach Urgent Care reserves the right to update these terms at any time. Continued use of our services constitutes acceptance of the revised terms.

Our accessibility standard and approach

Standard targeted: We use the Web Content Accessibility Guidelines (WCAG) Level AA as our benchmark for accessibility. We are continually improving our site with this standard in mind.
