3. Uses That Require Your Authorization
Except for the purposes noted above and other legally permitted uses, we will not use or disclose your PHI without your written authorization. You may later revoke an authorization unless it was already relied upon.

Access & Copies: Request access to and obtain a copy of your PHI in electronic or paper form.
Request Corrections: Ask us to amend your health information if you believe it is incorrect or incomplete.
Request Restrictions: Ask to restrict how we use or disclose PHI for treatment, payment, or operations — although we aren’t required to agree.
Accounting of Disclosures: Receive a list of certain disclosures we have made of your PHI.
Confidential Communications: Request that we contact you in a specific way or at a specific location.
File a Complaint: If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services (Office for Civil Rights) — and you will not be retaliated against for filing a complaint.

Where applicable and permitted by law, you may:

- Refuse to share certain information for purposes other than treatment, payment, or healthcare operations

Abide by the terms of this Notice as long as it remains in effect

We reserve the right to change this Privacy Notice and make the new terms effective for all PHI we maintain. Any updated notice will be posted on our website.
